in the Southern Tier will receive $16.5 million to improve long-term care services.

Lutheran Social Services in Chautauqua will receive $11.7 million while Wyoming County will receive $4.7 million, with funds coming for both through Phase 20 of the Health Care Efficiency and Affordability Law of New York State (HEAL NY) and the Federal-State Reform Partnership.

A total of nine health-care facilities and agencies statewide were selected to receive grants totaling more than $150 million to improve extended care services and develop community-based alternatives to traditional nursing home care for older New Yorkers.

Lutheran Social Services operates the Lutheran Retirement Home in Jamestown, doing business as Hultquist Place, which has received three HEAL NY grants in recent years totaling $23 million.

With 214 residential beds, the home on Falconer Street, received a $10.2 million grant last year to reduce the number of skilled beds and increase assisted-living beds. It also received funds to create a 40-slot adult day health-care program, which opened this summer.

The organization will announce additional details at an Oct. 1 press conference for its plans to create a full continuum of senior care services.

The HEAL NY Phase 20 competitive grants come through the State Department of Health and the Dormitory Authority of New York State to fund projects that maintain access to appropriately sized nursing home facilities while developing less restrictive options that New York’s seniors prefer.

In addition to maintaining existing nursing home beds, the grants support the development of assisted living residences and other housing configurations that provide coordinated medical services.
