Consultant Pharmacist - Los Angeles, California
We have an exciting new employment opportunity for an experienced corporate consulting Pharmacist with a well respected long-term care organization.
Requirements include:
- A minimum of two years of experience within Long Term Care
- Pharm. D. or Masters degree from an accredited School
- Current CA Pharmacist License
The Consulting Pharmacist will work collaboratively with both facility and pharmacy staff, review policies and procedures, and implement necessary changes with regard to;
- Review of physician orders for medication dosage, potential interactions, and route of administration.
- Maintenance of patient records to include medical history and concurrent use of medications including over-the-counter drugs.
- Documentation of communication with physicians, nurses, other members of the healthcare team and patients and their family members in the medical record.
- Maintenance of policies and procedures for the safe procurement, storage and distribution, and disposal of drugs.
